ABSTRACT

Introduction: Body piercings consist of small holes made with a needle in different parts of the skin or body to introduce a jewel or decorative element. In the oral cavity, most piercings are placed in the tongue. However, some complications may occur, and surgical techniques must be used for their removal. These complications present a certain degree of difficulty due to their position and may challenge the ability of the clinician to access the specific anatomical location. The different imaging techniques, from simple radiography to intraoperative techniques such as image intensifiers, have become an extremely useful tool for locating an object in the three dimensions of space, allowing safe location and extraction. Objective: The aim of this study is to report the case of a complication of a body piercing in the oral cavity and how the use of imaging was decisive for surgical planning and for the quick and effective resolution of the case. Material and Methods: A 14-year-old female patient came looking for treatment. Her mother reported the onset of the condition after the insertion of a needle-like metallic object while performing an artistic perforation in the lingual region. Since the girl was unable to extract the object, she sought medical advice at the Carlos Arvelo Military Hospital in Caracas, Venezuela. Subsequently, an imaging study was performed by means of a Computed Tomography to locate the metallic object. It was observed that the foreign body had migrated to the floor of the mouth/sublingual region, requiring the area to be surgically approached. It was also decided to use an intraoperative image intensifier. The removal of the object was performed satisfactorily. Conclusion: The extraction of foreign bodies placed in the lingual and sublingual region represents a challenge for the clinician due to the number of important anatomical structures that pass through that area. This makes clinicians plan their surgical removal using pre- and intraoperative imaging, to find a less traumatic location, reduce surgical time as well as the risk of damaging adjacent anatomical structures.


Introducción: Los body piercings consisten en producir perforaciones con una aguja en diferentes localizaciones de la piel con el fin de introducir una joya o elemento decorativo a través del agujero producido previamente. En la cavidad oral, la lengua es el sitio de mayor elección; sin embargo, en ciertas ocasiones suelen presentarse ciertas complicaciones, por lo cual se debe recurrir a técnicas quirúrgicas para su remoción presentando cierto grado de dificultad para su localización y la capacidad del clínico para acceder al espacio anatómico. Las diferentes técnicas imagenológicas desde una radiografía simple hasta técnicas intraoperatorias como los intensificadores de imágenes se han convertido en una herramienta sumamente útil para la ubicación de un objeto en las tres dimensiones del espacio, permitiendo una localización y extracción segura para el clínico y el paciente. Objetivo: El propósito de este trabajo es reportar un caso de una complicación de esta práctica en la región bucal y cómo el uso de la imagenología fue determinante para la planificación quirúrgica y la resolución del caso de manera rápida y efectiva. Material y Métodos: Se presenta a consulta una paciente femenina de 14 años de edad, quien madre refiere inicio de enfermedad actual posterior a introducirse objeto metálico tipo aguja de compás con la finalidad de realizar perforación artística en región lingual, al no poder extraer dicho objeto, acude a evaluación en el Hospital Militar Carlos arvelo de Caracas; posterior se realiza estudio imagenológico tipo Tomografía Computarizada para la ubicación del objeto metálico donde se observa que el cuerpo extraño habría migrado hacia el piso de boca/región sublingual, teniendo la necesidad de abordar quirúrgicamente la zona y elegir el uso de intensificador de imagen transoperatorio para retiro del mismo el cual se dio de manera satisfactoria. Conclusión: Los cuerpos extraños desplazados a la región lingual y sublingual representan un desafío para el clínico al momento de extraerlos, esto se debe a la cantidad de estructuras anatómicas importantes que pasan por dicha zona, lo que hace que el clínico opte en su planificación quirúrgica por el uso de imagenología pre y transoperatoria obteniendo así una localización menos traumática, menor tiempo operatorio y menor riesgo de lesión de estructuras anatómicas adyacentes.

ABSTRACT

Objective: To evaluate the effect of trifoliate flap design of radial forearm flap in reconstruction of defects after mouth floor cancer resection. Methods: From June 2016 to December 2019, 12 patients with defect after resection of mouth floor cancer were treated with trifoliate flap design of radial forearm flap. All of these patients were T2 stage, included 9 well-differentiated squamous cell carcinoma (SCC) and 3 moderate differentiated SCC. The defect size ranged from 8.0 cm×6.0 cm to 5.0 cm×4.5 cm after resection of tumor and neck dissection. All defects were repaired with trifoliate flap design of radial forearm flap. The flap size ranged from 8.0 cm×2.0 cm to 4.0 cm×1.5 cm, the donor site was sutured directly on Z plasty. Results: All flaps completely survived well. Both the wound and the donor site were stage Ⅰ healing. With the average follow-up of 38.6 months, the swallowing and speech function were satisfactory. Conclusions: Trifoliate flap design of radial forearm flap can effectively repair the postoperative defect of mouth floor cancer, and the donor site can be directly sutured on Z plasty. This technique can avoid forearm scar caused by skin grafting and the formation of the second donor site.

ABSTRACT

Dermoid cyst (DC) is a cystic lesion of developmental origin and uncertain etiology that rarely affects the floor of the mouth. We report a case of a large lesion found in the submental and submandibular region in a 25-year-old male patient. Computed tomography revealed extensive hypodense lesion in the submental and submandibular space without peripheral enhancement. The microscopical analysis showed a cystic cavity lined by orthokeratinized stratified squamous epithelium. The cystic capsule was composed of dense fibrous connective tissue containing cutaneous attachments, such as sebaceous and sweat glands. The diagnosis of DC was made. The differential diagnosis of expansive sublingual lesions can be clinically challenging due to the similarity with several lesions frequently observed in this region. Herein, we describe a case of extensive DC arising in the floor of the mouth, presenting clinical, imaging, and microscopical features.

Background: Hemorrhages, mouth floor edema and tongue elevation are complications related to surgical procedures in the anterior region of the mandible. Objective: The objectives of this study were to evaluate the presence and location of the lingual foramen in the anterior region of the mandible and to evaluate mandibular morphology using cone beam computerized tomography (CBCT). Material and method: The mandible's morphology and the location, diameter and height of the lingual foramina were analyzed using the midline and the mental foramen as references, in 278 CBCT. Result: 88% of the sample had a midline lingual foramen, totaling 408 foramina, with a mean diameter of 0.93 mm. Foramina in the lingual region between the midline and mental foramina were detected in 75% of the sample, with a mean diameter of 0.807 mm. There was no positive correlation between the presence of lingual foramina in the lateral or in the midline regions (r = -0.149; p = 0.013). In the midline region, the type I mandibular shape was predominant (96%), and type III was predominant in the lateral regions. Conclusion: Considering the prevalence of these structures and their clinical relevance in potential surgical complications, it is important to carefully analyze the anterior region of the mandible during surgical planning.


Introdução: Hemorragias, edema no assoalho bucal e elevação da língua são complicações relacionadas a procedimentos cirúrgicos na região anterior da mandíbula. Objetivo: Os objetivos deste estudo foram avaliar a presença e localização do forame lingual na região anterior da mandíbula e avaliar a morfologia mandibular utilizando tomografia computadorizada com feixe de cone (CBCT). Material e método: A morfologia da mandíbula e a localização, diâmetro e altura do forame lingual foram analisados utilizando a medula e o forame mental como referências em 278 CBCT. Resultado: 88% da amostra tinha um forame lingual da linha média, totalizando 408 forames, com um diâmetro médio de 0,93 mm. Na região lingual entre a linha média e forames mentais foram detectados em 75% da amostra, com um diâmetro médio de 0,807 mm. Não houve correlação positiva entre a presença de forames lingual nas regiões lateral ou na região média (r = -0,149; p = 0,013). Na região da linha média, a forma mandibular do tipo I era predominante (96%) e o tipo III predominava nas regiões laterais. Conclusão: Considerando a prevalência dessas estruturas e sua relevância clínica em possíveis complicações cirúrgicas, é importante analisar cuidadosamente a região anterior da mandíbula durante o planejamento cirúrgico.

Clavicle fracture, a very rare delayed complication following radical neck dissection of oral carcinoma, is normally ignored by oral and maxillofacial surgeons. We report and analyze a male patient with clavicle fracture after primary extended excision and bilateral radical neck dissection. This case was misdiagnosed as cervical metastasis.

ABSTRACT

Los Quistes Derrmoides son una patología frecuente, pero su localización en la cavidad oral es excepcional; cuando se presentan en el suelo de la boca muestran una tumoración benigna de la línea media. Para el abordaje quirúrgico son factores determinantes su tamaño y las relaciones anatómicas; sin embargo la exéresis intrabucal demuestra beneficios estéticos y funcionales. Se presenta el caso de una paciente de 9 años de edad, con aumento de volumen en el piso de la boca y región submentoniana, de 2 años de evolución, crecimiento lento, progresivo y asintomática. Se realizaron estudios complementarios y bajo anestesia general nasotraqueal, se procedió a su exéresis quirúrgica mediante un abordaje intrabucal. El diagnóstico histopatológico reportó Quiste Dermoide en el suelo de la boca. La paciente no ha tenido recurrencia de la lesión transcurridos 1 año de la cirugía.


The Derrmoid Cysts are a frequent pathology, nevertheless their location in the oral cavity is exceptional, when they occur in the floor of the mouth they show a benign tumor of the midline. Their size and anatomical location are determining factors in the surgical approach; however, the intra-oral exeresis demonstrates aesthetic and functional benefits. We present the case of a 9-year-old patient, with increased volume in the floor of the mouth and submental region, of 2 years of evolution, slow growth, progressive, asymptomatic. Complementary studies were carried out and under nasotracheal general anesthesia, we proceeded to surgical exeresis through an intra-oral approach. The histopathological diagnosis reported a Dermoid Cyst in the floor of the mouth. The patient had no recurrence of the lesion 1 year after the operation. A review is made about the diagnosis and surgical approaches of the Dermoid Cysts.

Objective @#To explore the effects of mouth floor approach intubation in the treatment of jaw fracture combined with skull base injury.@*Methods @#From Jan. 2014 to Oct. 2015, 7 cases of jaw fracture combined with skull base injury were selected from Medical center of Stomatology, Foshan Hospital of Traditional Chinese Medicine (FSTCM). All cases were treated with mouth floor approach intubation, because they were not suitable to be given the oral or nasal intubation anesthesia. @*Results @#All the patients healed well after operation, and there was no tube accident or other complications.@*Conclusions@# For patients of jaw fracture combined with skull base injury, the mouth floor approach intubation could be recommended as a supplementary choice for traditional intubation methods.

Abstract Hemorrhagic complications in oral anticoagulant therapy are frequent. Spontaneous sublingual hematomas secondary to anticoagulants are an extremely unusual complication that is potentially lethal. The study reports a case of a patient on anticoagulation therapy with warfarine who develops a massive, spontaneous sublingual hematoma with obstruction of the high airway and cardiac arrest, where emergency transtracheal catheter ventilation was used.


Resumo As complicações hemorrágicas do tratamento com anticoagulantes orais são conhecidas. Os hematomas sublinguais espontâneos secundários a anticoagulação são uma complicação extremadamente rara e potencialmente fatal. Descrevemos o caso de uma paciente anticoagulada com warfarina que apresentou um hematoma sublingual espontâneo massivo com obstrução da via aérea superior e parada cardiorrespiratória, e que foi submetida à ventilação transtraqueal de emergência.

ABSTRACT

Adenoid cystic carcinoma (ACC) of the sublingual gland is an extremely rare neoplasm. The clinicopathological characteristics of ACC are slow-growing swelling with or without ulceration, perineural spread, local recurrence, and distant metastasis. This report describes a 58-year-old male who had a slowly growing swelling without ulceration on the right side of the mouth floor that had been present for 1 month. In a radiological examination, the mass showed multilocular cystic features and no bony or tongue muscle invasion. No enlarged cervical lymph nodes were detected. Excisional biopsy and histological analysis showed that the lesion was ACC. In addition to reporting a rare case of ACC, this report also discusses the differential diagnosis and treatment of ACC with a review of the relevant literature.

O Cisto Dermóide é uma condição rara em região de cabeça e pescoço, caracterizado por apresentar um epitélio que contém estruturas anexas em sua parede, glândulas sebáceas, sudoríparas, folículos pilosos ou unhas. Quando em localização supra-milohióidea, o cisto dermóide pode causar deslocamento ântero-posterior da língua, causando transtornos fonéticos e respiratórios. O seu tratamento é a exérese cirúrgica, sendo a escolha do acesso cirúrgico dependente da localização da lesão e da experiência do cirurgião. Dermoid Cyst is a rare condition in head and neck area, histologically characterized by epithelium containing structures attached on its wall, sebaceous glands, sweat glands, hair follicles or nails. When located in upper-mylohyoid muscle area, the Dermoid Cyst can cause displacement of the tongue, respiratory and phonetic disorders. Its treatment is surgical excision, being the choice of surgical approach related to the lesion location and the surgeon's experience. This paper describes the clinical, pathological and imaging aspects as well as surgical treatment and follow up of this pathology. Introdução: O retalho miocutâneo infrahioideo (RMI) foi descrito por Wang em 1986. É utilizado em Cirurgia de Cabeça e Pescoço para reconstrução após ressecção de tumores, com sua principal indicação em tumores de andar inferior de boca. Objetivo: Avaliar, de forma retrospectiva, a exequibilidade e confiabilidade do RMI. Método: RMI foi utilizado na reconstrução cirúrgica em 25 pacientes portadores de carcinoma de andar inferior de boca. Apenas um dos cirurgiões do grupo tinha experiência prévia com a técnica. Os demais realizaram um único procedimento sob supervisão, a partir do qual os realizavam de forma independente. Resultados: Em três casos (12,0%) houve deiscência de sutura entre o retalho e o leito receptor, formação de fístula salivar e consequente deiscência de sutura no sítio doador. Em quatro pacientes (16,0%) houve epidermólise, com descamação e posterior reepitelização, sem necessidade de reintervenção cirúrgica.a Em dois pacientes (8,0%), houve necrose total da pele no terço distal do retalho, tratada com debridamento local. Não houve necrose muscular do RMI e não ocorreram complicações tardias. Comparando-se os três cirurgiões, não se observou diferenças nas taxas de complicações. Conclusão: O RMI é exequível no nosso meio, pois não demanda material específico e tem rápida curva de aprendizado. Os cirurgiões com vivência na especialidade podem facilmente incorporar essa técnica em sua rotina de reconstrução. As taxas de complicações são aceitáveis. O RMI é útil para reconstrução em cirurgia de cabeça e pescoço, notadamente para tumores iniciais de assoalho de boca e língua oral.


Introduction: The musculocutaneous infrahyoid flap (MIF) was described by Wang in 1986. It is used in reconstruction on head and neck surgery, with its main indication in tumors of the mouth floor. Objective: Evaluate retrospectively, the feasibility and reliability of the MIF. Method: The MIF was used in the surgical reconstruction in 25 patients with squamous cell carcinoma of the buccal floor. Only one surgeon of the group had prior experience with the technique. The other two surgeons performed the first procedure under supervision, then they performed independently. Results: Three cases (12.0%) had dehiscence between the flap and the recipient site, salivary fistula formation and subsequent wound dehiscence at the donor site. Four patients (16.0%) had epidermolysis with flaking and subsequent re-epithelialization without need for surgical intervention. In two patients (8.0%), there was complete necrosis of the distal skin flap treated with debridement site. There was no muscle necrosis of MIF and there were no late complications. Comparing the three surgeons , no differences in complication rates was observed. Conclusion: MIF is feasible in our country because it does not require special equipment and has fast learning curve . Surgeons with experience in the art can easily incorporate this technique into your routine reconstruction. Complication rates are acceptable. The MIF is useful for reconstruction of head and neck surgery, especially for tumors initial floor of the mouth and oral tongue.

ABSTRACT

During surgical procedures, unexpected material, including surgical instruments and tissue segments, may get lost in the surgical field. Most of these should be immediately removed to prevent further complications, such as vital organ irritation, infection, and inflammatory pseudo-tumor formation. However, it is not always easy to define the exact location of the foreign body, especially if the item is very small and/or it is embedded in the soft tissue of the head and neck region. Intraoperative real-time radiological imaging with C-arm fluoroscopy can be useful to trace the three-dimensional location of small and embedded foreign bodies in the oral and maxillofacial area. We describe an unusual case of an embedded micro-screw in the intrinsic tongue muscle that had been dropped into the sublingual space during a lower alveolar bone graft procedure. The lost foreign body was accurately identified with C-arm fluoroscopy and safely removed without any further complications.

ABSTRACT

A ranula is a bluish, transparent, and thin-walled swelling in the floor of the mouth. They originate from the extravasation and subsequent accumulation of saliva from the sublingual gland. Ranulas are usually limited to the sublingual space but they sometimes extend to the submandibular space and parapharyngeal space, which is defined as a plunging ranula. A 21-year-old woman presented with a complaint of a large swelling in the left submandibular region. On contrast-enhanced CT images, it dissected across the midline, and extended to the parapharyngeal space posteriorly and to the submandibular space inferiorly. Several septa and a fluid-fluid level within the lesion were also demonstrated. We diagnosed this lesion as a ranula rather than cystic hygroma due to the location of its center and its sublingual tail sign. As plunging ranula and cystic hygroma are managed with different surgical approaches, it is important to differentiate them radiologically.

ABSTRACT

ObjectiveTo evaluate the clinical value of infrahyoid myocutaneous flap in the reconstruction of mouth floor tissue defects. MethodsInfrahyoid myocutaneous flap was made including stenohyoid,thyrohyoid,omohyoid and the affiliated skin based on the axis of superior thyroid artery,vein and ansa cervicalis. The clinical data of 11 patients with reconstruction of mouth floor tissue defects using infrahyoid myocutaneous flap (maximum area was 8 cm × 3 cm) from Match 1993 to June 2009 were retrospectively reviewed.ResultsSeven infrahyoid myocutaneous flaps which reserved platysma branch of superior thyroid artery were all alive, while 2 patients presented epidermal necrosis of 4 un-protected patients. The follow-up period was range from 24 months to 195 months, one patient received salvage surgery for local regional lymphatic recurrence in level Ⅰ,which survived without diseases until now.The rest cases had turned out a success.No dysphagia,dysphonia,and tongue extension difficulty was reported in any patient.ConclusionInfrahyoid myocutaneous flap is one of the valuable donor sites for the reconstruction of mouth floor tissue defects after radical oncologic operations,especially for those with early stage carcinoma of the mouth floor.

ABSTRACT

Schwannoma is a relatively slow-growing, encapsulated benign tumor that is derived from the Schwann cell of the nerve sheath. We report here on a case of schwannoma of the mouth floor with a review of the literature. A 67-year-old woman presented with a right mouth floor mass, which was first detected 5 years ago. The preoperative diagnosis was ranula on the basis of the physical findings and the computerized tomographic findings. However, the mass was found to be a true neoplastic lesion rather than a cystic lesion in the course of surgical dissection. The permanent pathologic report of the mass was schwannoma. Postoperatively, although the patient had no problem with taste, the pain-sense, speech and swallowing, she had mild deviation of the tongue towards the same side of the mass, which means that the function of the hypoglossal nerve was somewhat impaired. Her tongue deviation was spontaneously resolved within 6 weeks postoperatively.

ABSTRACT

El quiste dermoide representa menos del 0,01 por ciento de todos los quistes de la cavidad bucal. Su ubicación más frecuente es en suelo de boca. Se presenta el caso de una paciente femenina de 19 años de edad que hacía aproximadamente 7 años había notado un aumento de volumen debajo de la lengua, el cual le crecía paulatinamente y ya se apreciaba por fuera de la cara. Además, le ocasionaba molestias al hablar y al masticar. Se realizaron estudios complementarios y bajo anestesia general nasotraqueal. Se procedió a su exéresis quirúrgica mediante un abordaje intrabucal, donde se obtuvieron excelentes resultados estéticos y funcionales. El diagnóstico histopatológico se correspondió con un quiste dermoide de suelo de boca. La paciente no ha tenido recurrencia de la lesión transcurridos tres años de la operación. El quiste dermoide de suelo de boca se presenta como una tumoración benigna de la línea media. La exéresis intrabucal demuestra beneficios estéticos y funcionales(AU)


The Dermoid cyst account for the 0.01 percent of all cysts of buccal cavity. Its more frequent location is in the mouth floor. This is the case of a female patient aged 19 who approximately 7 years noted an increase of volume under tongue growing gradually and noting outside face and the discomfort at to speak and to chew. Complementary studies were conducted and under general anesthesia a surgical exeresis was carried out by intrabuccal approach achieving excellent esthetic and functional results. Histopathologic diagnosis matched with a dermoid cyst of mouth floor. Patient has not lesion recurrence after three years after operation. We conclude that the Dermoid cyst of mouth floor appear as benign tumor of middle line. The intrabuccal exeresis demonstrates esthetic and functional benefits(AU)

ABSTRACT

Introdução: O cisto dermoide é um cisto de desenvolvimento incomum na face e envolve mais frequentemente o assoalho bucal. A maioria das lesões ocorre em adultos jovens, com ligeira predileção pelo gênero masculino. As lesões neonatais e em crianças são extremamente raras. Manifesta-se como tumefação flutuante, assintomática e de crescimento lento e progressivo, atingindo dimensões variadas. Sua cápsula pode conter um ou mais anexos cutâneos, como glândulas sebáceas, pêlos ou unhas. O tratamento é cirúrgico, através de enucleação, sendo sua recidiva incomum. O aspecto clínico, histopatológico, diagnóstico diferencial e tratamento de um caso de cisto dermoide congênito, e de sua recidiva, são discutidos pelos autores.


Introduction: The dermoid cyst is a cyst of unusual development in the face and more often involves the mouth floor. Most injuries occur in young adults with a slight predilection for males. Lesions in neonatal and children are extremely rare. It manifests as swelling floating asymptomatic and slow-growing and progressive, reaching dimensions. His capsule may contain one or more skin appendages such as sebaceous glands, hair or nails. The treatment is surgical, by enucleation, and its recurrence uncommon. The clinical appearance, histopathology, differential diagnosis and treatment of a case of congenital dermoid cyst, and its recurrence, are discussed by the authors.
